Social Work and Case Management
At Sonora Regional Medical Center, social workers and case managers are available to patients of all ages. The Social Services Department has three full-time, master’s-trained social workers prepared to help patients and families manage the medical, emotional, and social issues related to their illness or injuries. The Medical Center’s three full-time case managers are specially trained registered nurses who provide information and support for patients and their families.
To help in coordinating medical care for individual patients, the case managers and social workers assess the patient’s physical and emotional needs and arrange for services to meet those needs. They also help develop discharge plans and may also contact patients and their families after discharge to evaluate continuing care needs.
